Job summary

HammondCare is seeking an experienced Transformation Lead to plan, communicate, coordinate and embed transformation across initiatives in a 22-month term at St Leonards with hybrid working options. The role focuses on preparing leaders and teams for change, aligning stakeholders and monitoring adoption to ensure outcomes.

You will work with initiative owners, business leaders and Marketing to ensure understanding of transformation, readiness and uptake across services.

Qualifications

• Commitment to HammondCare’s Christian Mission and values.
• 10+ years of demonstrated experience in organisational transformation.
• Ability to assess change impacts across people, processes, systems and ways of working.
• Experience developing and coordinating change, communication and stakeholder engagement activities.
• Experience supporting leaders and managers to prepare for and lead transformation.

Responsibilities

• Stakeholder Engagement & Leader Support: help leaders and stakeholders understand, prepare for and lead transformation.
• Change Impact & Readiness: assess effects on people and processes and ensure support is in place before change is implemented.
• Communication & Awareness: deliver clear communications to build alignment and confidence in transformation initiatives.
• Implementation & Adoption Support: coordinate activities to transition teams to new processes and enable adoption after go-live.
• Adoption Monitoring & Reinforcement: measure adoption outcomes and address barriers to sustained transformation.
• Enterprise Transformation Visibility: maintain an organisation-wide view of transformation activity and risks.
• Continuous Improvement: capture lessons learned to strengthen future transformation approaches.
