Job summary

St Vincent’s Hospital Sydney is seeking a Specialist Pharmacist – Inflammatory Bowel Disease (IBD) to provide high-quality pharmaceutical care across inpatient and ambulatory care. You will work with the IBD MDT, educate patients on immunosuppressants and biologics, and lead pharmacotherapy clinics with gastroenterology supervision.

The role is a max term, part-time contract (20 hours/week) in the Gastroenterology department, Sydney, with pro-rata salary and benefits including super and salary

Qualifications

• Current Pharmacy Registration with AHPRA and no conditions on scope of practice.
• Ability to educate and counsel patients on high-risk medications including immunosuppressants, biologics and targeted therapies.
• Strong understanding of regulatory, formulary management and PBS prescribing requirements.
• Commitment to development of specialist IBD pharmacy services with multidisciplinary teams.
• Excellent written and verbal communication skills for guidelines, policies and education.

About the role

• Provide high-quality pharmaceutical care to IBD patients across inpatient and ambulatory settings, working collaboratively with the multidisciplinary IBD team to deliver a safe, effective and cost-effective pharmacy service

• Deliver education and counselling to patients receiving immunosuppressive therapies, including immunomodulators, biologics and targeted oral therapies

• Support shared decision-making by ensuring patients understand the benefits, risks and monitoring requirements of IBD medications

• Participate in therapeutic drug monitoring clinics and IBD multidisciplinary team (MDT) meetings, providing expert advice on IBD therapies

• Manage a pharmacist-led virtual immunomodulator clinic on Tuesday and Friday afternoons and Thursday mornings, including pathology review and dose adjustment under gastroenterologist supervision

• Develop and implement protocols for the prescribing, administration and monitoring of IBD therapies

About you

• Current Pharmacy Registration with AHPRA with no conditions on scope of practice and able to meet the criteria set out in the Named NSW (Non-Declared) Affiliated Health Organisations Employees Agreement 2019 for a Grade 2 Pharmacist

• Ability to assess, develop, implement and evaluate the education and counselling needs of patients receiving high-risk medications, immunosuppressants, biologics and targeted therapies

• Strong understanding of regulatory, medication governance, formulary management and funding processes, including PBS prescribing requirements, biologic approvals, compassionate access programs and medication safety principles

• Commitment to the development, implementation and advancement of specialist IBD pharmacy services in collaboration with multidisciplinary healthcare teams

• Highly developed written and verbal communication skills, including the ability to prepare clinical guidelines, protocols, policies, educational resources and research outputs of a high professional standard
