Job summary

South Eastern Sydney Local Health District (SESLHD) is seeking a Social Worker Level 1-2 at Prince of Wales Hospital. Plan and deliver high quality social work care to patients and families, aligned with SESLHD policies.

Role requires psychosocial assessments, casework and crisis intervention with a commitment to continuous professional development. Temporary Full Time position until October 2027, 38 hours per week, with salary range $79,419–$113,301 plus superannuation.

Qualifications

  • Qualifications per NSW Health Service Health Professionals Award with eligibility for AASW.
  • Experience in psychosocial assessments, casework, crisis intervention, counselling and discharge planning.
  • Knowledge of community resources and liaising with community organisations.
  • Strong written/verbal communication and interpersonal skills.
  • Ability to work in a multi-disciplinary team and exercise independent judgement.
  • Computer skills and use of health information systems.

Responsibilities

  • Plan, coordinate and deliver high quality social work care to patients, families and carers.
  • Work across a seven day roster including weekends and participate in After Hours Crisis On-Call.

Plan, coordinate and deliver high quality Social Work care to patients, families and carers consistent with South Eastern Sydney Local Health District (SESLHD) policies, procedures and standards.

Where you'll be working

Prince of Wales Hospital and Community Health Service is a Level 6 Tertiary Referral Hospital with an inpatient bed base of 370. Each year we care for more than 70,000 patients in our Emergency Department and have around 50,000 admissions to the inpatient units. POWH offers an outpatient service and rural outreach service and provides more than 900,000 occasions of non-admitted patient care each year, including innovative virtual models of care. We have transitioned the majority of inpatient units and wards to the Acute Services Building (ASB) including a state of the art Emergency Department and Intensive Care Unit. POWH provides an exceptional staff experience and enhanced patient and family outcomes as a result of the provision of person-centred care.

Selection Criteria

  • Qualifications in accordance with Schedule C of the NSW Health Service Health Professionals (State) Award providing eligibility for membership of the Australian Association of Social Workers.
  • Demonstrated core competency skills in psychosocial assessments, individual and family casework, crisis intervention, counselling, advocacy and discharge planning.
  • A good knowledge of community resources and experience liaising with community organisations.
  • Demonstrated high level written and verbal communication, and interpersonal skills, and the ability to proactively engage with patients/clients to enhance service delivery.
  • Ability to work as part of a multi-disciplinary team and to exercise independent professional judgement on routine matters, commensurate with years of experience
  • Computer skills and the capacity to use health information systems.
  • Demonstrated commitment to quality improvement and evidence based practice and a commitment to ongoing clinical and professional learning

Current drivers licence (or a willingness to travel in accordance with the demands of the position) and the ability to participate in the After Hours Crisis On-Call service, and work across a seven day roster (including weekends).
