Job summary

SA Health's Northern Adelaide Rehabilitation Service at Modbury Hospital is seeking Social Workers at AHP1/2 levels to help patients navigate recovery and transition home. You will perform psychosocial assessments, support families through changes, and discharge planning within a supportive, multidisciplinary team.

You will engage in goal setting with patients, connect them to community supports such as NDIS and aged care, and maintain meticulous documentation while contributing to service

Qualifications

• Experience in hospital, rehabilitation or health-related setting.
• Understanding of psychosocial assessment, discharge planning and care coordination.
• Knowledge of SA community services including NDIS and aged care.
• Strong interpersonal and communication skills; ability to build relationships.
• Ability to work in a multidisciplinary team and contribute to patient outcomes.
• Strong problem-solving and organisational skills; manage competing priorities.
• Commitment to culturally safe, inclusive and person-centred practice.

Responsibilities

• Complete psychosocial assessments and provide interventions to support patients during rehabilitation.
• Support patients and families through change, adjustment and complex decisions.
• Collaborate with patients, families and the multidisciplinary team to identify goals and plan safe discharge.
• Connect patients with community supports including NDIS pathways, aged care and other services.
• Maintain high-quality clinical documentation per SA Health standards.
• Contribute to service development, quality improvement and positive patient outcomes.
