Job summary

Barwon Health in Geelong is seeking a dedicated social worker for a permanent part-time role across diverse health settings. You will deliver patient-centred social work services within multidisciplinary teams, supporting patients and families through transitions and complex care needs.

Qualifications

About the role

Join our social work team in a permanent part time role supporting patients and their families across a diverse range of health settings.

You’ll work as part of a supportive social work department and multidisciplinary team, collaborating closely with nurses, doctors and other allied health professionals to provide patient-centred care.

This role will deliver a social work service primarily in the Transitional Care Program (TCP) and Barwon Health’s Residential Aged Care facilities.

Key responsibilities

• Provide high-quality social work assessment and intervention

• Work collaboratively within multidisciplinary teams
• Contribute to safe, patient-centred care across a range of health settings

What we're looking for
