Job summary

Barwon Health is seeking a permanent part-time social worker to support patients and families across acute, subacute and community health settings in Geelong. You will join a supportive social work team and work within a multidisciplinary group to deliver patient-centred care.

About the role

Join our social work team in a permanent part time role supporting patients and their families across a diverse range of health settings.

You'll work as part of a supportive social work department and multidisciplinary team, collaborating closely with nurses, doctors and other allied health professionals to provide patient-centred care.

This role will deliver a social work service primarily in the Transitional Care Program (TCP) and Barwon Health's Residential Aged Care facilities.

Key responsibilities

• Provide high-quality social work assessment and intervention
• Support patients and families with complex health and social needs
• Work collaboratively within multidisciplinary teams
• Contribute to safe, patient-centred care across a range of health settings

What we're looking for

• A relevant Social Work qualification and eligibility for membership of AASW
• Clinical social work experience, preferably within a health setting
• Strong assessment, communication and interpersonal skills
• Ability to work collaboratively within multidisciplinary teams
• Flexibility to work across different health settings and campuses
