Latrobe Regional Health in Gippsland is seeking a Social Worker (Grade 1) to join our dynamic Social Work team on a full-time ongoing basis. You will provide ethical, person-centred psychosocial care within a multidisciplinary healthcare environment, under supervision, focusing on wellbeing, recovery and safe discharge planning.

Qualifications

• Bachelor or qualifying Masters in Social Work and eligibility for full membership with the Australian Association of Social Workers (AASW).
• Strong knowledge of social work principles, ethics and legislation within a healthcare environment.
• Excellent communication and interpersonal skills; ability to build relationships with patients, families and healthcare professionals.

Responsibilities

• Complete psychosocial assessments to identify risks, strengths and social factors affecting care and discharge planning.
• Provide counselling, advocacy, support and discharge planning in collaboration with patients, families and carers.
• Collaborate with multidisciplinary teams to support coordinated, person-centred care and participate in case conferences and family meetings.
• Maintain accurate documentation in electronic medical records and departmental systems.
• Engage in supervision, reflective practice and professional development.
