cohealth in Melbourne's CBD is seeking a Social Worker to join a multidisciplinary team delivering primary health care and harm reduction services to people with complex needs. You will provide short-term casework, psychosocial assessment and care planning, assess needs, advocate for clients, and coordinate services across sectors to improve health and social outcomes.

This position is based across cohealth's CBD services at 244 Flinders Street, which provide integrated primary health care and harm reduction support to people who use drugs, with a strong focus on improving health and social outcomes for individuals experiencing complex needs.
