Job summary

Ramsay Clinic Northside – St Leonards Park NSW is seeking a Social Worker to join our team on a permanent, full-time basis. You will deliver recovery-focused psychosocial care across community and onsite settings, supporting patients’ mental health, wellbeing and independence.

You will work with families, carers and a multidisciplinary team to achieve recovery goals, ensure safe transitions, and advocate for patients with comprehensive social supports.

Qualifications

• Bachelor of Social Work (or recognised equivalent).
• Minimum 4 years experience as a Social Worker.
• Eligible for full membership of Australian Association of Social Workers.
• Experience in Healthcare environments.

Responsibilities

• Provide psychosocial assessments and risk assessments with evidence-based social work interventions.
• Support patients to identify and achieve recovery goals and strengthen connections with family and community.
• Deliver therapeutic interventions including brief interventions, counselling, motivational interviewing and trauma-informed practice.
• Collaborate with multidisciplinary team to develop care plans and coordinate transitions and discharge planning.
• Provide advocacy, education and practical support to patients, carers and families.
• Address adjustment, loss, grief and social impacts of mental illness with patients and families.
• Identify and respond to safeguarding concerns including child/adult safeguarding.
• Develop partnerships with community organisations to connect patients with services.

An exciting opportunity is available to become an integral member of our team as aSocial Workeron apermanent, full-time basis.

In this role, you will provide high-quality, recovery-focused psychosocial care to patients, supporting their mental health, wellbeing, independence and connection with family, community and other support services.

Working across the community and onsite clinic, you will collaborate closely with patients, families, carers and the multidisciplinary team to provide person-centred care, support recovery goals and facilitate safe and effective transitions across services.

Responsibilities:

• Provide comprehensive psychosocial assessments, risk assessments and evidence-based Social Work interventions for patients experiencing a range of mental health presentations.
• Support patients to identify and achieve individual recovery goals, build independence and strengthen their connection with family, community and social supports.
• Provide therapeutic interventions including brief interventions, supportive counselling, motivational interviewing and trauma-informed practice, where appropriate.
• Work collaboratively with the multidisciplinary team to develop and review individualised care plans and facilitate safe, coordinated transitions and discharge planning.
• Provide advocacy, education and practical support to patients, carers and families.
• Support patients and families experiencing adjustment, loss, grief, bereavement and the social and psychological impacts of mental illness.
• Identify and respond appropriately to risk and safeguarding concerns, including child abuse, elder abuse and domestic and family violence.
• Develop strong partnerships with community organisations and external services to connect patients with appropriate health, housing, employment, education and social supports.
• Contribute to multidisciplinary case discussions, clinical reviews and collaborative care planning.
• Maintain high standards of professional practice, clinical documentation, confidentiality and ethical decision-making.

About You

To be successful in this role, you will meet the following criteria:

• Bachelor of Social Work (or recognised equivalent).
• Minimum 4 years experience as a Social Worker
• Eligible for full membership of Australian Association of Social Workers.
• Demonstrated ability to utilise a variety of Social Work interventions, inclusive of effective problem solving andethical decision -making skills.
• Demonstrated experience in Healthcare environments
• Knowledge of community resources to meet client’s needs.
• Proven ability to work independently and effectively in a multidisciplinary team.
• Well-developed interpersonal and communication skills (verbal and written)
• Familiar with Microsoft Suite
• Unrestricted NSW drivers' licence
• Personal vehicle that may be used for work purposes
