Job summary

Matilda Nepean Hospital in Western Sydney is seeking an experienced full-time social worker to join our team, working closely with a multidisciplinary group including a discharge planner.

Applicants should be AASW registered with 1–2 years post-graduate experience in mental health, rehabilitation, or palliative care, and be skilled in evidence-informed practice, communication, and team working.

Qualifications

• AASW Registration is required.
• 1–2 years post-graduate experience.
• Experience in Mental Health, Rehabilitation and/or Palliative Care.
• Experience managing clients with complex needs.
• Experience managing NCAT, NDIS, MAC and other services.
• Evidence-informed practice and critical thinking.
• Strong verbal and written communication.
• Ability to work independently and as part of a cohesive team.

Responsibilities

• Provide social work services for mental health, rehabilitation and palliative care clients.
• Collaborate with the multidisciplinary team including a discharge planner to coordinate care.
• Support smooth patient discharge and transition to appropriate services.

Criteria:

AASW Registration

Minimum of 1-2 years of post graduate experience

Experience working in Mental Health, Rehabilitation and or Palliative Care.

Experience managing clients with complex needs

Experience managing NCAT, NDIS, MAC and other services.

Demonstrated understanding of evidence informed practice and critical thinking

Sound verbal and written communication skills

Ability to work independently and as part of a cohesive team
