The role involves short-term counselling, developing person-centred care plans, and contributing to quality improvement. A recognised Social Work degree and AASW eligibility are required, with a current driver licence and required

Working as part of a multidisciplinary team, you will support clients, families and carers through assessment, counselling, advocacy and care planning, helping improve health and wellbeing outcomes across our community.Key ResponsibilitiesDeliver evidence-informed Social Work services across hospital, aged care and community-based programs.Apply a Social Model of Health approach to assess client needs and develop person-centred care plans.Provide short-term counselling for clients experiencing chronic disease, rehabilitation, mental health and other challenges.Advocate for clients, carers and families to ensure access to appropriate supports and services.Participate in multidisciplinary case conferences and collaborative care planning.Maintain accurate clinical records and contribute to service development and continuous quality improvement.Build and maintain productive relationships with health professionals, community providers and support agencies.About youTo be successful in this role, you will have:A recognised university qualification in Social Work.Eligibility for membership with the Australian Association of Social Workers (AASW).Current Driver Licence.Current NDIS Worker Screening Check and Working with Children Check, or ability to obtain.Excellent communication and interpersonal skills with the ability to develop strong working relationships.Experience working across a range of Social Work practice areas, particularly with older people.Experience working within multidisciplinary teams to deliver person-centred services.Strong counselling skills and a commitment to evidence-informed practice and professional development.Please refer to the relevant attached position description and address the key selection criteria on your cover letter when applying.Culture and BenefitsLocated a short drive from Bendigo and Melbourne, Heathcote is a renowned centre for its regional food and wine culture.
