Job summary

Metro North Hospital & Health Service in Caboolture, QLD, is seeking a Social Worker to join the Department of Social Work Services. The role supports inpatients, outpatients, families and carers facing acute psychosocial trauma, aiming to improve health and wellbeing outcomes.

Job description

The Social Worker provides professional social work services as part of the multidisciplinary team within the Department of Social Work Services at Caboolture and Kilcoy Hospitals. The role supports inpatients, outpatients, families and carers experiencing acute psychosocial trauma, complex psychosocial issues or circumstances that impact their health and wellbeing, helping them achieve the best possible outcomes from healthcare services.

The successful candidate will deliver assessment, counselling and intervention services, contribute to multidisciplinary case management, and provide support across a range of acute clinical areas. The role also supports service improvement initiatives, professional education, research activities and the ongoing enhancement of patient-centred care across the Caboolture, Kilcoy and Woodford Directorate.
