Responsibilities

• Coordinate care and services for residents with complex needs.
• Manage home resources like assets and vehicles.
• Implement and interpret complex support plans.
• Oversee risk management and incident reporting.
• Maintain accurate documentation and stakeholder relationships.
• Provide on-call support occasionally.
