Job summary

360 Health Management is seeking dedicated Support Workers to join our regional disability service team across Shepparton and surrounding areas. You will support participants with daily living activities, personal care, meal preparation and community participation, including sleepovers in a SIL house.

Ideal candidates hold a Certificate III in Disability or related field, have a valid Australian driver's license, and demonstrated experience in similar roles.

Qualifications

• Experience in a similar role
• Unrestricted Australian Driver's Licence and access to a vehicle (may be required for community roles)
• Certificate III in Disability Support or related field is preferred

Responsibilities

• Work directly with clients supporting daily living activities, including personal care, meal preparation, and household tasks.
• Support clients in accessing community resources and participating in social activities.
• Complete daily shift notes detailing client progress and steps toward goals.
• Complete incident reports and detail any changes in behaviour or health to supervising staff.
• Collaborate with other healthcare professionals and attend care team meetings.
• Foster boundaries with participants and engage in capacity-building activities.

Here at 360 Health Management, we pride ourselves in being a strong regional disability support provider servicing Shepparton, Bendigo, Swan Hill and Seymour. We provide a range of services to our NDIS participants including Direct Community Access and in Home Support, to Support Coordination, Behaviour Support, Allied Health and much more.

We are looking for dedicated and reliable Support Workers with previous experience who are eager to work with participants of all ages and needs. With shifts covering all hours of the day, including sleepovers in a SIL house.

Responsibilities:

• Work directly with clients supporting with daily living activities, including personal care, meal preparation, and household tasks.
• Support clients in accessing community resources and participating in social activities.
• Complete daily shift notes to accurately detail client progress and steps towards their goals
• Complete incident reports and detail any changes in behaviour or health to the supervising staff.
• Collaborate with other healthcare professionals to ensure comprehensive care for clients which may include attending care team meetings.
• Foster strong boundaries with participants and engage in other capacity building activities.

What you will bring:

• A Certificate III or above in Disability Support, Individual Support or other related fields.
• Unrestricted Australian Drivers Licence and access to a reliable car
• Previous demonstrated experience in a similar role.
• Strong communication and interpersonal skills to effectively engage with clients and their families.
• Ability to work independently and as part of a team, demonstrating flexibility and adaptability.
• A valid Australian driver's licence and access to a vehicle may be required for community support roles.
