North Coast Aboriginal Corporation For Community Health (NCACCH), trading as Manngoor Dja Aboriginal Health Services, invites applications for a Social Worker – Social Emotional Wellbeing (SEWB) to join our Gympie team. The role focuses on delivering culturally appropriate mental health interventions, case management and referral pathways within a multidisciplinary team.

Qualifications

• Accredited Mental Health Social Work or 3+ years social work experience, preferably in Mental Health.
• Experience in an Aboriginal and Torres Strait Islander ACCHO or similar.
• Experience providing follow-up care for at-risk or chronically ill/mentally ill patients.
• Supervising and coordinating teams; planning, delegation.
• Strong project management; able to meet deadlines under pressure.
• Person-centred, respectful, empathetic and culturally sensitive.
• Commitment to teamwork and strong community relationships.

Responsibilities

• Provide mental health intervention practices, case management and referrals.
• Lead care coordination within the SEWB program.
• Oversee development and performance of the SEWB Officer and team.
• Collaborate with the Health Services team to improve Indigenous health outcomes.
