Wellways seeks a Service Integration Coordinator for Connected Care in Tasmania. You will map health and community services, build referral pathways, and engage networks to connect people to the right support. The role supports a statewide service and involves collaboration with multiple organisations and teams.
You’ll work across the North and North-West regions, travel regularly, and contribute to a no-wrong-door model while prioritising culturally safe practice and recovery-oriented care.
At Wellways, we celebrate diversity and are committed to creating an inclusive workplace where everyone feels valued, respected and empowered to contribute. We warmly welcome applications from Aboriginal and Torres Strait Islander peoples, people with lived experience of mental health challenges or disability, people from multicultural backgrounds, and people who identify as LGBTIQA+. We recognise that diverse experiences and perspectives strengthen our workforce and the communities we support.
As Service Integration Coordinator, you’ll help establish a new statewide mental health service by building relationships and referral pathways that connect people to the right support at the right time. You’ll map health and community services, connect with local organisations, support community engagement and maintain the Connected Care Support Catalogue.
This is a practical, relationship-focused role with variety and the opportunity to help shape how a new ‘no wrong door’ service works across Tasmania.
Connected Care is a statewide mental health service delivered by Wellways in partnership with Stride Mental Health and Clarity Health Care and commissioned by Primary Health Tasmania.
The service supports adults aged 18 years and over to access free, coordinated mental health and wellbeing support, using a stepped-care approach that responds to their individual needs and goals. A multidisciplinary team brings together clinical, psychosocial and peer expertise to support people to access the right care and move between levels of support as their needs change.
The role can be based in Tasmania’s North or North-West, with regular travel across the region and some travel elsewhere in Tasmania.
You’ll bring experience building strong working relationships across health, mental health or community services, with the ability to connect people and organisations and support the development of practical referral pathways.
You’ll have an understanding of community development and recovery-oriented, trauma-informed and strengths-based approaches, along with knowledge of the broader services that support people's mental health and wellbeing.
You’ll also bring:
