Job summary

NSW Health invites applications for a Social Worker (Level 3) within the Community Palliative Care team at Royal Prince Alfred Hospital. This permanent full-time role (38 hours per week) provides social work support to community clients accessing palliative care.

You will join a dedicated Social Work Department, work in a multidisciplinary setting, participate in after-hours on call, and contribute to end-of-life care, grief and family support, with strong opportunities for professional

Qualifications

• Bachelor or postgraduate degree in Social Work with eligibility for full membership of the Australian Association of Social Workers (AASW).
• Experience at Level 2 or higher in social work, including palliative care or end-of-life settings.
• Strong verbal and written communication, and interpersonal skills for working with diverse patients and families.
• Ability to work independently and within a multidisciplinary hospital team, with ethical boundaries.

Responsibilities

• Provide social work support to people in the community accessing palliative care.
• Collaborate within the Social Work Department in a hospital setting to support patients and families.
• Participate in after-hours on-call crisis service, including weekends.
• Contribute to bereavement support, care planning, and discharge planning as required.
