Job summary

BlueCare in Mt Isa and surrounding areas is seeking a Senior Personal Carer to build meaningful relationships and deliver person-centred support in clients' homes and communities. You will conduct intake and screening, develop and review individualised care plans, and provide daily living assistance, transport and social support while maintaining accurate records in CRM/CMS.

A valid Cert III Aged Care and driver's licence are essential.

Qualifications

• Must have Certificate III Aged Care / Individual Support or equivalent.
• Current Australian driver's licence (Green Provisional minimum).
• Vehicle to use for work - roadworthy & insured (Third Party Property minimum).
• Experience with technology including smartphones, tablets, laptops.

Responsibilities

• Conduct initial client intake and screening assessments and develop care plans.
• Provide client-centred care for personal self-care, housekeeping, social support, respite, transport and shopping assistance.
• Manage client information in CRM & CMS and keep case notes up to date.
