Job summary

BlueCare is seeking a Senior Personal Carer to provide in-home and community care across Springwood to Meadowbrook. You will conduct intake, build care plans and support clients with daily activities while working within our Not-for-Profit framework.

Essential qualifications include a Certificate III Aged Care and a current Australian driver’s licence. A vehicle is required, and travel to client homes is part of the role.

Qualifications

• Certificate III Aged Care or equivalent is essential.
• Current Australian Driver’s Licence is essential.
• Roadworthy vehicle to use for work is essential.
• Experience with smartphones, tablets, laptops etc is preferred.
• Ability to pass probity checks as required by policy.

Responsibilities

• Take initial client intake and screening assessments.
• Develop and review individualised care plans with a client-centred approach.
• Maintain client information in CRM & CMS and keep case notes up to date.
• Engage in ongoing care discussions, planning, reviews and goal evaluation.
• Travel to client homes and support daily activities.
• Support with community activities and transport/shopping as needed.
