BlueCare is seeking a Senior Personal Carer to conduct initial client intake, assess needs, and develop personalised care plans. Role involves on-site visits across Toowoomba North, Oakey & surrounding areas and coordination with the care team to review goals and outcomes.

You will travel to clients’ homes, provide self-care support and transportation assistance, and engage in community activities in line with the Care Plan. Requires Certificate III Aged Care and a valid driver’s license.

Qualifications

• Certificate III Aged Care, Individual Support or equivalent - essential.
• Current Australian Driver's Licence (Green Provisional minimum) - essential.
• Roadworthy & insured vehicle to use for work - essential.
• Experience with technology incl. smartphones, tablets, laptops etc.
• Confidence to satisfy probity checks required by legislation or policies.

Responsabilités

• Conduct initial client intake and screening assessments and develop client-centred care plans.
• Manage client information in CRM & CMS; keep notes up to date.
• Engage in ongoing care discussions, planning, assessment and reviews.
• Travel to clients' homes for scheduled visits and assist with daily activities.
• Provide care per the individual Care Plan and assist with community activities.
• Assist with transportation and shopping and appointments.

About the role:

As a Senior Personal Carer you'll conduct initial client intake and screening assessments and developing and reviewing individualised care plans with a client-centered approach while providing quality care based on clients' needs.

• Conduct thorough initial screening assessments, developing care plans for personal self-care, general house cleaning, social support, respite, transport and shopping assistance.
• Management of client information in CRM & CMS ensuring all details and case notes are kept up to date
• Engage in ongoing care discussions, planning, assessment, reviews, and evaluating goals and outcomes in consultation with the care team and as required.
• Spending your time on the road completing scheduled visits, interacting one-on-one with clients within their homes supporting with daily activities
• Providing care in accordance with their individualised Care Plan
• Supporting with community based recreational and social activities
• Transportation and assistance with shopping and appointments.
