• Advocate for participants and support them to exercise choice, control and access to appropriate services.
• Manage a caseload of NDIS participants while delivering high-quality Support Coordination.
• Maintain accurate documentation, reports and case notes in line with NDIS Practice Standards and organisational requirements.
• Assist participants to navigate NDIS, prepare for plan reviews and maximise plan outcomes.
• Coordinate, review and oversee service delivery to ensure supports are meeting the participants requirements and goals.
• Build participant’s capacity and confidence.
• Provide expert advise and consultation to NDIS participants and their families around navigating NDIS.
• Build strong relationships with participants, families, providers and key stakeholders.
• Identify and manage participant risks or concerns.
