Beenleigh & Districts Senior Citizens Centre in Beenleigh, QLD seeks an experienced Support at Home Case Manager to deliver person-centered case management for older clients under the Support at Home Program.

You will assess needs, coordinate in-home and community services, and manage client budgets while ensuring compliance with aged care standards. Strong communication and independence are essential, with familiarity of AlayaCare and Australian aged care guidelines.

Formación

• Minimum 2 years' experience in case management or care coordination within aged care, community, or health sectors.
• Strong understanding of the Australian Aged Care system, including Support at Home/Home Care Packages.
• Experience in preparing and managing client budgets and using client management systems.

Responsabilidades

• Undertake comprehensive assessments to develop individualised care plans.
• Coordinate and monitor the delivery of in-home and community services.
• Review and adjust care plans to stay effective and relevant.
• Build strong relationships with clients, families and service providers.
• Advocate for clients' needs and independence.
• Prepare, manage and monitor client budgets per program guidelines.
• Maintain accurate documentation and ensure compliance with aged care standards.
• Utilise AlayaCare to manage client records and service delivery.
