About the role

As the Senior Clinical Care Partner – Support at Home, you will bring your hands‑on experience in aged care Support at Home or community aged care to provide senior clinical leadership and oversight across BallyCara’s Support at Home program.

This is an opportunity to use your knowledge of delivering care to older people in their own homes to support high-quality, person-centred services that promote independence, wellbeing and choice.

You will play a key role in ensuring clients receive care that is aligned with their goals, assessed needs, clinical requirements and available funding, while supporting our team to deliver safe, effective and compliant services.

This is a full-time, Monday to Friday role, based at our Kippa Ring office, reporting to the Integrated Community Manager, who is based in our Melbourne office.

You will be part of a connected Support at Home team, while having the autonomy to manage your day-to-day work locally and independently.

This is a senior clinical role that combines clinical leadership, care oversight, service coordination, mentoring and clinical governance. You will work closely with Care Partners and the broader multidisciplinary team to support integrated service delivery across nursing, allied health and care supports.

While this role is not intended to hold a full caseload, there may be times when you provide direct client support or manage a small caseload to support leave coverage or service continuity.

What you will be responsible for

  • Providing senior clinical leadership and oversight across BallyCara’s Support at Home services
  • Using your aged care community and in‑home care experience to support safe, person‑centred and goal‑focused outcomes for clients
  • Supporting and mentoring Care Partners and the wider team to deliver high-quality care and services
  • Overseeing complex referrals, clinical assessments, care planning, reviews and service coordination
  • Supporting clients with complex and changing needs to remain safely and independently at home
  • Monitoring client funding, care management activity and service delivery to ensure services align with client goals and program requirements
  • Providing guidance and support around clinical risk, escalation and changes in client needs
  • Driving clinical governance, documentation quality, continuous improvement and alignment with aged care legislation and quality standards
  • Working collaboratively with nursing, allied health and care teams to deliver coordinated and integrated services
  • Building strong relationships with clients, families, carers, internal teams and external stakeholders
  • Supporting the ongoing development and growth of BallyCara’s Support at Home services
Desired Skills and Experience

About you

We are looking for an experienced Registered Nurse with strong, hands‑on experience in aged care Support at Home, community aged care or a similar in‑home care environment.

You understand what it takes to support older people to live safely and independently at home and are confident navigating the complexities of community‑based care, including clinical assessment, care planning, service coordination, risk management and changing client needs.

You will bring a calm, practical leadership style, sound clinical judgement and the ability to balance high-quality, person‑centred care with clinical governance, compliance and operational requirements.

Most importantly, you will bring:

  • Current AHPRA registration as a Registered Nurse
  • Demonstrated experience working in aged care Support at Home, Home Care Packages, community aged care or a similar in‑home care environment
  • Strong understanding of the delivery of aged care services to clients living in their own homes
  • Practical knowledge of aged care funding, care management and service coordination
  • Strong understanding of the Support at Home Program and the Strengthened Aged Care Quality Standards
  • Experience conducting clinical assessments, developing care plans, coordinating referrals and managing complex client needs
  • Experience working collaboratively with clients, families, carers, support workers, allied health professionals and external service providers
  • Demonstrated leadership experience, including coaching, mentoring and supporting team capability
  • The ability to identify and respond to clinical risk and changes in a client's condition or circumstances
  • Excellent communication and relationship‑building skills
  • Strong organisational skills and the ability to manage competing priorities while maintaining high-quality documentation
  • Sound clinical judgement, resilience and the ability to navigate change and solve problems in a practical, client‑focused way
