Job summary

Southern NSW Local Health District is seeking a Senior Aboriginal Health Worker for the Aboriginal Chronic Care Program based at Mount Druitt Hospital. You will support Aboriginal and Torres Strait Islander people to access culturally safe chronic care services and act as a trusted link between communities, healthcare providers and support services.

Qualifications

• This identified Aboriginal position requires knowledge of Aboriginal cultures and strong community links.
• Holds or is willing to undertake Certificate III in Aboriginal Health Care or equivalent qualifications.
• Experience with NSW Aboriginal Health Programs or social support and health promotion with Aboriginal communities.
• Ability to lead culturally safe environments and provide cultural brokerage.

What you'll be doing

As the Senior Aboriginal Health Worker within the Aboriginal Chronic Care Program, you will play a pivotal role in supporting Aboriginal and Torres Strait Islander people to access culturally safe chronic care services. You will act as a trusted link between the community, healthcare providers and support services, helping individuals and families navigate their healthcare journey while strengthening service responsiveness to community needs.

• Support Aboriginal people and their families to access community, primary, specialist and hospital-based chronic care services
• Build trusted relationships and develop referral pathways that improve access to coordinated care and support
• Advocate for culturally safe healthcare practices and enhance service capability to meet the needs of Aboriginal communities
• Deliver education, health promotion, community engagement and practical support for people living with chronic and complex conditions
• Foster collaboration between WSLHD services, Aboriginal Community Controlled Organisations, government agencies and community partners

Selection Criteria

1. 1. This is an identified Aboriginal Position. Applicants must be of Aboriginal and / or Torres Strait Islander descent, have demonstrated knowledge and understanding of Aboriginal and / or Torres Strait Islander cultures and have established links with their local Aboriginal community. An applicant’s race is a genuine occupational qualification and exemption is claimed under Section 14d of the Anti-Discrimination Act 1977, NSW.
2. 2. Holds or is willing to undertake a minimum Certificate III qualification in Aboriginal Health Care or other relevant qualifications (including undergoing recognition of prior learning processes against current qualifications)
3. 3. Experience in NSW Aboriginal Health Programs and / or experience in providing social support, linkage to services, group work, community education, and health promotion with the Aboriginal community
4. 4. Demonstrated understanding of issues impacting on Aboriginal and Torres Strait Islander people and their carers / families and the impact that chronic and complex conditions may have
5. 5. Demonstrated capacity to lead the development of culturally safe and respectful environments for Aboriginal clients / patients and their families
6. 6. Demonstrated ability to provide cultural brokerage with Aboriginal people and communities; cultural consultancy for other members of the team; and to facilitate respectful and culturally safe access to services.
7. 7. Demonstrated ability to work autonomously and as a member of a multidisciplinary team with effective interpersonal, problem solving and computer skills
8. 8. Current drivers licence valid in NSW
