The purpose of the position is to work collaboratively with health care service providers across the LHD to increase access to health treatment and prevention services for Aboriginal people as well as addressing issues relating to the management of chronic disease with an added focus area to improve the patient journey and outcomes for Aboriginal people within their communities.

As an AHW/ AHP you will follow a Practice Plan of Care under the supervision and direction of a registered Clinical Supervisor. Activities will be specific to the "Chronic Disease for Aboriginal People" Model of Care, to support the care of Aboriginal people with or at risk of developing chronic disease/s. This includes delivery of an Integrated Care focus providing follow up services, care navigation or coordination, health education and cultural support.

The AHP role will also provide a range of direct clinical services to meet the needs of Aboriginal people at risk of chronic disease/s, as defined by an agreed Practice Plan of Care and primary healthcare functions.

To do this, the position will provide specific functions such as:

  • Initiating, undertaking assessments, tests and procedures
  • Working collaboratively in an integrated approach with hospitals and other chronic disease services to ensure Aboriginal Chronic Care Unit (ACCU) initiatives improve the patient journey from hospital to community
  • Working within a multidisciplinary health team/s, under direct or indirect supervision, either individually or in a team or group, using routine primary health care practices and procedures and established techniques or methods.

From Helensburgh in the north to North Durras in the south, ISLHD covers an area of 5,687 square kms. That’s 5,687 square kms of beautiful beaches, coastal cycling tracks, lush rainforests, incredible bushwalks and friendly people. The region is also host to many annual food, wine and arts festivals and events.

ISLHD operates seven hospital sites, employ over 8,500 staff and provide community health services across 58 locations. Commute times are short, childcare and schooling options are plentiful, and Sydney and Canberra are close by.
