Mallee Track Health & Community Service – Ouyen VIC is seeking a Rural Outreach Worker to provide person-centred, non-clinical support. You will help people experiencing vulnerability access services, navigate systems and receive short-term assistance and referrals where needed.

The role focuses on information sharing, practical support and carers through the Carer Support Program, without counselling or ongoing case management.

Qualifications

• Diploma of Community Service or equivalent, or willingness to work towards.
• Mental Health First Aid qualifications or willingness to obtain.
• Ability to build rapport with diverse individuals and organisations.
• Capacity to work independently and in a team.

Responsabilités

• Provide person-centred, non-clinical support to vulnerable people.
• Assist navigation and access to relevant services and supports.
• Deliver information, practical support and referrals to carers.
• Maintain privacy and confidentiality and accurate records.
• Collaborate with the team and other services to improve wellbeing.
