Northern Health in Melbourne's northern suburbs is seeking a Grade 1 Social Worker for a rotation position across inpatient wards. The role provides clinical social work services, psychosocial assessments and discharge planning under supervision, rotating every 6 months.

The successful candidate will join a multidisciplinary team and support patients and families in diverse communities. Permanent full-time role with opportunities to contribute to patient-centred care and professional development

Qualifications

• Experience of undertaking psychosocial assessments & putting appropriate interventions in place.
• Proven ability to address stressful clinical and/or non-clinical situations appropriately.
• Demonstrated clinical skills at a new graduate Grade 1 level of competence, at least.
• Well-developed written communication skills as evidenced in documentation and written reports.
• Effective interpersonal skills e.g. problem solving, decision making, etc.

Responsabilités

• Provide quality patient-centred care through the application of competent clinical social work skills in assessment, planning, implementation of treatment and discharge planning.
• Provide clinical social work services to patients as required, with appropriate supervision within the Social Work team.
• Rotate into various clinical roles ~6 months per rotation, on inpatient wards at The Northern Hospital Epping, as part of a multidisciplinary team.
• Respond to inpatient referrals and provide psychosocial assessments and interventions to patients and their families.
