Bayside Health is recruiting a Registered Nurse - Community Mental Health to deliver acute, recovery‑oriented care in the community across Melbourne and surrounding suburbs. This Hospital in the Home (HITH) role supports consumers in their homes, with supervision by a multidisciplinary team and a focus on preventing relapse and unnecessary admission.

Qualifications

• Current registration as a Registered Nurse (Division 1) with AHPRA.
• Postgraduate qualifications in Mental Health Nursing (or equivalent).
• Domestic candidates: 3+ years’ full-time mental health experience; international: 4+ years.
• Driver's Licence.

Responsabilités

• Deliver advanced, person‑centred, and recovery‑oriented mental health nursing care in the home.
• Conduct mental state and psychosocial assessments and develop crisis/relapse plans.
• Provide psychoeducation, medication management, and supportive counselling.
• Monitor physical health, liaise with GPs and other health providers.
• Offer timely crisis intervention and coordinate admissions when needed.
• Coordinate care, discharge planning, and linkage with community supports.
• Maintain contemporaneous clinical documentation and meet governance standards.
• Collaborate with a multidisciplinary team to ensure holistic care.
