Me Medical Recruitment seeks a Registered Nurse with mental health expertise to deliver acute, recovery-oriented care in a Hospital in the Home (HITH) service launching in Melbourne. Work autonomously within a multidisciplinary team to support consumers in their homes and community settings.

The role requires postgraduate mental health nursing qualifications and 3+ years (domestic) or 4+ years (international) of relevant experience, with sponsorship available for eligible applicants.

Qualificações

• Current registration as a Registered Nurse (Division 1) with the Australian Health Practitioner Regulation Agency (AHPRA).
• Postgraduate qualifications in Mental Health Nursing (or equivalent recognised specialisation - UK Mental Health Nursing Qualification is sufficient).
• Domestic Candidates: Minimum of three (3) years full-time equivalent experience in acute, community, or crisis intervention mental health settings.
• International Candidates: Minimum of four (4) years full-time equivalent experience in acute, community, or crisis intervention mental health settings. UK, Ireland and NZ experienced highly regarded.
• Driver's License.

Responsabilidades

• Deliver advanced, person-centered, and recovery-oriented mental health nursing care in the home setting.
• Conduct comprehensive mental state and psychosocial assessments, formulate clinical risk assessments, and develop crisis/relapse prevention plans with the consumer and family/carers.
• Provide a range of evidence-based therapeutic interventions, including psychoeducation, medication management, and supportive counselling.
• Conduct physical health assessments and observations, monitor effects of mental illness and psychotropic medications, liaise with GPs and other health providers.
• Provide timely crisis intervention and support, facilitating admissions only when clinically indicated.
• Contribute to seamless care coordination, discharge planning, and linkage with community supports and continuing care services.
• Maintain accurate, timely clinical documentation in the EMR and ensure compliance with Mental Health and Wellbeing Act 2022 and clinical standards.
• Collaborate with the multidisciplinary team to ensure holistic and coordinated care.
