Mercy Health is recruiting an experienced Registered Nurse (Mental Health) for its Hospital in the Home (HITH) service launching in Melbourne. This role delivers acute, recovery‑oriented mental health care in the community, enabling avoidance of inpatient admission.

You will work autonomously within a multidisciplinary team to provide evidence‑based care to consumers in their homes, including assessments, risk management, crisis support and care coordination with families and GPs.

Qualifications

• Current registration as a Registered Nurse (Division 1) with AHPRA.
• Postgraduate qualification in Mental Health Nursing.
• Minimum five years post‑graduate experience in aged mental health and community care.

Responsabilités

• Deliver advanced, person‑centred, and recovery‑oriented mental health nursing care in the home setting.
• Conduct comprehensive mental state and psychosocial assessments, formulate clinical risk assessments, and develop crisis and relapse prevention plans.
• Provide evidence‑based therapeutic interventions, including psychoeducation, medication management, and supportive counselling.
• Conduct physical health assessments and observations, liaise with GPs and health providers.
• Provide timely crisis intervention and support, facilitating admissions only when clinically indicated.
• Contribute to seamless care coordination, discharge planning, and linkage with community supports.
