Job summary

Transform Life Pty in Australia is seeking an experienced Occupational Therapist to join our national allied health team. You will deliver evidence-based OT assessments and ongoing therapy to NDIS participants, complete standardised assessments, therapy plans and NDIS reports in a flexible, home-based setting.

Workload is shaped by your preferred age groups, with travel typically within a 30-minute radius, telehealth as needed, and a supportive multidisciplinary team.

Qualifications

• Occupational Therapy qualification and full AHPRA registration.
• NDIS experience (ideally 12 months).
• Suitable home-based working environment and WWCC.
• NDIS Worker Screening Check and a valid driver’s licence.

Responsibilities

• Deliver evidence-based OT assessment and ongoing therapy to NDIS participants with physical and mental health needs.
• Complete functional and standardised assessments, therapy planning and NDIS reports.
• Shape your caseload around your preferred age groups.
• Travel within a 30-minute radius of home, with telehealth used where appropriate.
• Collaborate with families, support coordinators and other stakeholders to translate recommendations into practical outcomes.
• Work with multidisciplinary team while managing reports and admin from home or in the field.
