• Understanding of Aboriginal cultural values.
• Commitment to self-determination of Aboriginal communities.

Aufgaben

• Provide evidence-based, culturally appropriate education to clients and their carers across the KWHB region to support the recognition of health risks, diabetes management and the self-management of chronic disease, while building trusting and respectful relationships with individuals, families and communities.
• Work collaboratively alongside Aboriginal Health Coaches to support coordinated, culturally responsive chronic disease care, including client education, self-management support, health promotion, follow-up and ongoing engagement with clients and families.
• Provide education and counselling using an empowerment approach and education to clients, carers, and families of those living with diabetes.
• Liaise with other members of the Primary Health Care team together with the client to identify any barriers to supporting clients in the management of diabetes and develop strategies to address these.
• Support and participate in quality improvement, data collection and reporting activities to support the provision of safe, effective, and high-quality health care.
