Healthcare Australia (HCA) is seeking a capable Care Manager for Homecare My Way, based in Warragul and delivering services in a virtual capacity. You will manage client relationships, develop care plans, and ensure high-quality service delivery in line with funding.

You will collaborate with care teams, support onboarding and business development, and maintain compliant documentation while promoting exceptional client and workforce experience.

Qualifications

• Strong understanding of Aged Care and/or related health services.
• Experience in customer-focused environments with strong communication.

Responsibilities

• Develop care plans that support quality outcomes in line with funding.
• Manage client caseload and virtual service delivery.
• Ensure client documentation and case notes are up to date and compliant.
• Collaborate with teams on onboarding and related policies.
