Job summary

Mind Australia is seeking a Community Mental Health Practitioner - Relief worker to provide recovery-focused support in a 24/7 residential setting across Rockhampton and Yeppoon. The role involves sleepovers, weekends, and public holidays as part of a rotating roster.

You will help clients transition from hospital-based care back into the community, delivering high-quality support to promote independence, wellbeing and social connections in a safe environment.

Qualifications

• Experience working with people with mental health challenges and complex needs.
• Ability to implement recovery-oriented frameworks and practices.

Responsibilities

• Support people transitioning from acute mental health care back into the community.
• Deliver recovery-oriented support promoting independence, wellbeing, and social connection.
• Facilitate individual and group activities aligned with clients' recovery goals.
• Work collaboratively with families, carers, clinical services and community partners.
• Maintain accurate documentation and a safe, supportive residential environment.
