Mind Australia is seeking a Community Mental Health Practitioner for Rockhampton & Yeppoon. You will provide recovery-focused support in a two-house residential setting, working a 24/7 rotating roster including sleepovers, weekends and public holidays.

Qualifications

• Certificate IV or higher qualification in Mental Health or related field.
• Experience working with people with mental health challenges and complex needs.
• Current First Aid and CPR certificates.
• Current Blue card and NDIS Workers Screening (or willing to obtain).
• Vaccination proof or willingness to obtain required vaccinations.

Responsibilities

• Support people transitioning from acute mental health care back into the community.
• Deliver recovery-focused support promoting independence, wellbeing and social connection.
• Facilitate individual and group activities aligned with recovery goals.
• Work with families, carers, clinical services and community partners.
• Maintain accurate documentation and contribute to a safe residential environment.

About the role

As a Community Mental Health Practitioner, you will provide recovery-focused support to people living in a residential setting as they transition from hospital-based care back into the community.

This role is based across two residential houses in Rockhampton and Yeppoon and involves working a 24/7 rotating roster, including sleepovers, weekends, and public holidays.

Key Responsibilities

• Support people transitioning from acute mental health care back into the community.
• Deliver high-quality, recovery-oriented support that promotes independence, wellbeing, and social connection.
• Facilitate individual and group activities aligned with clients' recovery goals.
• Work collaboratively with families, carers, clinical services, and community partners.
• Maintain accurate documentation and contribute to a safe, supportive residential environment.

About the service

The Transitional Recovery Program (TRP) provides short-term residential support for adults transitioning from acute mental health care back into the community. Operating across two residential houses in Rockhampton and Yeppoon, the service offers a safe, home-like environment where people can focus on recovery, develop skills for independent living, and build meaningful connections with their community.

What you’ll bring

• Certificate IV or higher qualification in Mental Health, Community Services, Disability, Youth Work, Psychology, Social Work, or a related field (or willingness to work towards obtaining one).
• Experience working with people with mental health challenges and complex needs, including families and carers.
• Proven capability to implement therapeutic, recovery-oriented frameworks and practices including risk mitigation, trauma- informed care and family inclusive practice.
• Current First Aid and CPR certificates.
• Current Blue card and NDIS Workers Screening check (or willingness to obtain)
• Ability to provide proof of vaccination against vaccine preventable diseases or willingness to obtain required vaccinations per Queensland Health partnership.
