Mind Australia is a leading mental health organisation offering casual opportunities across Rockhampton and Yeppoon to support individuals transitioning from acute care to community living. The role sits in a two-house TRP setting with a 24/7 rotating roster, including sleepovers and holidays.

As a Community Mental Health Practitioner, you will help clients develop daily living skills, connect with community resources and participate in recovery-focused activities, while collaborating with

Qualifications

• Must have Certificate IV or higher in Mental Health or a related field.
• Experience with people with mental health challenges and complex needs.
• Ability to implement recovery-oriented practices including risk mitigation and trauma-informed care.
• Current First Aid and CPR certificates.
• Blue card and NDIS Workers Screening, or willingness to obtain.
• Vaccination proof or willingness to obtain per Queensland Health requirements.

Responsibilities

• Support people transitioning from acute mental health care back into the community.
• Deliver recovery-oriented support that promotes independence, wellbeing and social connection.
• Facilitate individual and group activities aligned with clients' recovery goals.
• Work collaboratively with families, carers, clinical services, and community partners.
• Maintain accurate documentation and contribute to a safe, supportive residential environment.
