Job summary

Southern Cross Care Limited in Young, NSW is seeking a Registered Nurse (Division 1) to assess, plan, and manage nursing care for residents in an aged care facility.

You'll develop personalised care plans, conduct evaluations, and ensure all quality documentation and funding submissions are accurate and timely. The role requires at least 2 years of RN aged care experience and current AHPRA registration.

Qualifications

• RN with AHPRA registration and Division 1 status.
• Minimum 2 years' RN experience in an aged care facility.
• Knowledge of ANACC assessments and funding submissions.

Responsibilities

• Carry out initial and ongoing assessment, planning, and management of nursing care for residents
• Develop personal care plans to identify and meet residents' needs, and monitor changes in care plans
• Participate in quarterly care plan evaluations, or more frequently as needed
• Ensure all documentation for quality care is prepared, maintained, monitored, evaluated, and up to date in accordance with SCC policies and procedures
• Ensure completion and accuracy of all ACFI claims

Job Description

About the Candidate

• Registered Nurse (Division 1) with general AHPRA registration
• Minimum 2-years' experience as an RN within an Aged Care Facility
• Ability to set and maintain high standards in the provision of quality care and support services
• Ability to provide quality care and support to frail aged people
• Sound understanding of relevant State and Commonwealth legislation as it applies to Aged Care Providers
• Availability to work a variety of shifts including morning, afternoon, night, and weekend shifts
• Understanding of ANACC, including assessments, documentation, and funding submissions
