Job summary

Southern Cross Care NSW & ACT is seeking a dedicated Registered Nurse to join our Home Care Team on a part-time basis in the Central Coast region. You will deliver high-quality, person-centred home care, supporting independence and quality of life for older Australians in their homes.

The role offers flexibility with three days per week (Wednesday, Thursday and Friday), ongoing training, and a supportive not-for-profit workplace with strong values and career growth opportunities.

Qualifications

• Registered Nurse (Division 1) with general registration.
• Certificate III in Community Services (Aged Care) or equivalent (desirable).
• Experience in caring for older people.
• Sensitivity to and understanding of the aspirations and needs of frail older people.
• Excellent interpersonal and written communication skills.
• Advanced verbal and written English communication skills.

Responsibilities

• Conducting initial and ongoing assessments, planning and managing nursing care.
• Developing and reviewing individualised care plans to meet residents’ changing needs.
• Participating in quarterly care plan reviews (or more frequently where required).
• Maintaining accurate, up-to-date documentation in line with SCC policies and quality standards.
• Ensuring the completion and accuracy of ACFI claims.
• Supporting best practice clinical care in a collaborative team environment.
