Job summary

Illawarra Shoalhaven Local Health District is seeking a Refugee Health Nurse to join the Refugee Health team within the Multicultural Health Service. The role focuses on care coordination, referrals and education across refugee health services in a diverse regional setting.

The position supports coordinated care for refugees, collaborates with GPs and providers, and contributes to data reporting and service improvement across multiple locations including Warrawong.

Qualifications

  • Registered nurse with current AHPRA licence.
  • Minimum of 3 years post-registration experience in cross-cultural community health.
  • Post-registration qualifications in refugee health or related field preferred.

Responsibilities

  • Care coordination, proactive follow up, referrals and health advocacy for refugee clients with complex needs.
  • Provide advice and support to health providers for refugee health care.
  • Support community engagement for refugee communities and individuals.
  • Maintain data, systems and reporting for stakeholders.
  • Coordinate and/or deliver education to GPs and health workers on refugee health.

The Refugee Health Nurse works with the Refugee Health team within the Multicultural Health Service in the following areas:

  • Care coordination, proactive follow up, referral and health service advocacy for refugee clients with multiple and/or complex needs in conjunction with general practitioners and relevant service providers.
  • Provision of advice and support to key health service providers in delivery of best practice care for people from refugee like backgrounds.
  • Support delivery and coordination of community engagement for refugee communities and individuals.
  • Maintenance of data, systems and reports to key stakeholders.
  • Coordination and/or provision of education and training for GPs, other health professionals and non-health workers and organisations in relation to refugee health.

From Helensburgh in the north to North Durras in the south, ISLHD covers an area of 5,687 square kms.   • Registered nurse with a current licence to practice from the Australian Health Practitioner Regulation Agency (AHPRA).
