The Adult Community Health Service provides multidisciplinary, person-centred care across a range of community-based programs and clinical services. These include Care Coordination, Community Rehabilitation, Post-Acute Care, Rapid Response Falls, the Queensland Ambulance Service Falls Co-Responder service, Pulmonary Rehabilitation, Persistent Pain Management, Refugee Health, Aboriginal and Torres Strait Islander Health Worker services.

The multidisciplinary Rapid Response Falls Service provides a community based integrated pathway for older adults at risk of falls who are living in the Cairns region to access rapid in-home assessment, tailored interventions, and linkages to existing services to reduce risk of falls and reduce conveyance to emergency departments.

About your role:

Your main tasks may include (but are not limited to):

• Comprehensive in-home nursing assessments, clinical observations and deterioration management.
• Falls risk assessment and prevention.
• Wound, skin integrity and clinical device care.
• Care coordination, referrals and clinical handover.
• Multidisciplinary assessment and care planning.
