Sundale Ltd. is seeking a Registered Nurse to join our Home Care team on the Sunshine Coast. The In Home Care Coordinator will coordinate and oversee individualized services to help clients live safely and independently at home.
You will conduct clinical assessments, develop care plans, manage budgets and mentor team members to ensure high-quality, person-centered care within our community programs.
Qualifications
- Registered Nurse qualification with AHPRA registration.
- CPR certificate current.
- QLD Driver Licence current.
- Vehicle suitable and insured.
- National Police Check completed.
- Pre-employment medical assessment completed.
Responsibilities
- Conduct clinical assessments and develop client care plans.
- Provide ongoing case management and adjust plans as needed.
- Monitor client needs and adapt plans as circumstances change.
- Lead and support In‑Home Care Carers and Domestic Assistants.
- Collaborate with GPs, allied health and external providers.
- Assist clients navigating My Aged Care processes.
- Participate in audits, quality improvement and accreditation.

We’re seeking an experienced Registered Nurse to join our Home Care team in an In-Home Care Coordinator position, supporting the delivery of safe, high-quality and individualised care across our community-based programs.

As a Registered Nurse working in our In Home Care Coordinator position, you will play a key role in coordinating and overseeing individualised services that support clients to live safely and independently in their own homes. You’ll use your Registered Nursing expertise to conduct clinical assessments, develop care plans, manage budgets, mentor team members, and ensure care is person‑centred, safe and high‑quality.
- Conduct comprehensive assessments and reviews, and develop tailored Care Plans
- Provide ongoing case management in line with funding and clinical requirements
- Monitor client needs and adjust plans as circumstances change
- Lead, mentor and support In‑Home Care Carers and Domestic Assistants
- Collaborate with internal teams, GPs, allied health and external providers
- Support clients to navigate My Aged Care processes
- Participate in audits, quality improvement and accreditation activities
- Ensure person‑centred care aligned with wellness, dignity and cultural respect
- Provide clinical oversight consistent with Registered Nursing standards
- Registered Nurse qualification & AHPRA registration
- Current CPR certificate
- Current QLD Driver Licence
- Safe, reliable and appropriately insured vehicle
- National Police Check
- Pre‑employment medical assessment
- Experience in care coordination, case management or similar roles
- Strong knowledge of aged care programs, Quality Standards, ageing and dementia
- Excellent communication, empathy and cultural awareness
- Skilled in care management / CRM systems
- High-level clinical judgement and understanding of chronic disease management
- Ability to work autonomously and collaboratively within a multidisciplinary team
- Above‑award wage rates
