Latrobe Regional Health is seeking a part-time Registered Nurse (HITH) to join our Home as Hospital in the Home (HITH) team. The role supports delivery of hospital-level care in patients’ homes and in the HITH Clinic, combining autonomous practice with collaborative, multidisciplinary care.

The role offers fixed-term 0.4EFT for 12 months, with opportunities for professional development and a range of staff benefits as part of LRH.

Qualifications

• Current registration as a Division 1 Registered Nurse with AHPRA.
• A Victorian Driver Licence is required.
• Ability to work autonomously and in multidisciplinary teams.

Responsibilities

• Deliver acute and subacute nursing care to Home Hospice patients in their homes and HITH clinic.
• Administer medications and infusion therapies within the HITH setting.
• Conduct endocrine dynamic testing and other specialised clinical procedures as required.
• Support patients receiving post-acute care nursing services and other Tier 2 community programs.
• Collaborate with medical, nursing and allied health professionals to coordinate care and optimise patient outcomes.
• Maintain safe work practices and comply with safety requirements and risk assessments.

Latrobe Regional Health is looking for Registered Nurse (HITH) to join our growing on a part time (0.4EFT) fixed term basis for 12 months (multiple positions available).

The Registered Nurse - Hospital in the Home (HITH) delivers high-quality acute, subacute and infusion-based care to patients in their homes and within the HITH Clinic setting. As part of the Hospital in the Home (HITH) team at Latrobe Regional Health, you will support the delivery of patient-centred care that enables patients to receive hospital-level treatment in the comfort and safety of their own residence.

The role of the Registered Nurse (HITH) is to:

• Deliver acute and subacute nursing care to admitted HITH patients in their homes, ensuring safe, effective and patient-centred care.
• Administer medications and infusion therapies within the HITH Clinic and community settings.
• Conduct endocrine dynamic testing and other specialised clinical procedures as required.
• Support patients receiving post-acute care nursing services and other Tier 2 community-based programs.
• Collaborate with medical, nursing and allied health professionals to coordinate care and optimise patient outcomes.
• Maintain safe work practices and comply with community nursing safety requirements, including patient risk assessments and personal safety protocols.

About you

Ideal candidates for this role will have:

• Current registration as a Division 1 Registered Nurse with AHPRA.
• A current Victorian Driver Licence (or equivalent).
• Strong clinical assessment and decision-making skills, with a commitment to delivering high-quality patient-centred care.
• Excellent communication and interpersonal skills, with the ability to build effective relationships with patients, families and multidisciplinary teams.
• The ability to work independently and manage a varied caseload across community and clinic settings.
• A flexible, adaptable and compassionate approach to care delivery.
