Are you a confident clinical leader who can support a team to achieve meaningful client outcomes, manage complex care needs, and work collaboratively with allied health professionals?

We are looking for someone who leads by example, maintains high professional standards and approaches challenging situations with composure, sound judgement and constructive communication. You will be confident managing complex clinical situations, supporting your team through challenges and making decisions that prioritise client safety and quality of care.

About Right at Home

We are passionate about delivering high-quality care and building strong, trusted relationships with our clients, their families and the broader healthcare community.

Based in Mitchell, ACT, we service Canberra's north side and surrounding NSW areas.

About the role

Reporting to the Managing Director, the Clinical Care Partner role combines clinical nursing, client care management, team leadership and clinical oversight.

You will work closely with our nurses, care workers, clients, families, GPs and allied health professionals to coordinate care, manage complex clinical needs and support the achievement of individual client goals.

This role requires a strong clinical foundation, excellent communication skills and the ability to provide clear direction and support to a diverse care team. You will help foster a culture of accountability, collaboration, continuous improvement and person-centred care.

Key responsibilities
  • Provide clinical leadership, guidance and support to our clinical team and community care workers.

  • Develop, review and oversee individualised care plans that reflect client goals, preferences, clinical needs and funding arrangements.

  • Support the management of clients with complex care needs, including palliative care, dementia, wound care, catheter care and diabetes management.

  • Work collaboratively with allied health professionals, GPs and other healthcare providers to coordinate services and achieve positive client outcomes.

  • Conduct nursing assessments and visits in the community, monitoring client wellbeing and responding to changing care needs.

  • Oversee clinical documentation, care planning, incident management and compliance with relevant policies and professional standards.

  • Support, mentor and develop nurses and care workers to promote safe, consistent and high‑quality care.

  • Manage challenging situations involving clients, families and team members with professionalism, empathy, resilience and sound clinical judgement.

  • Lead by example through accountability, integrity, respectful communication and consistently high professional standards.

  • Liaise with prospective clients and their families to understand their needs and develop appropriate care solutions.

  • Monitor client budgets and service delivery to ensure services align with approved funding and care requirements.

  • Manage competing priorities, maintain effective communication and ensure timely follow‑up of client and team enquiries.

  • Build and maintain professional relationships with clients, families, healthcare providers and the broader community.
