Job summary

HammondCare in Australia is seeking a part-time Clinical Partner - Nursing to support clients living at home in the Central West Region (Bathurst, Lithgow, Oberon, Cowra). This permanent role is 3 days per week, Monday to Friday, requiring you to drive between clients' homes.

As a Registered Nurse with current AHPRA registration, you will work with a multidisciplinary team to provide tailored and person-centred care, assess needs, set goals, and support independence for older adults living at

• Strong communication skills and ability to plan and manage time effectively.

About the role

This is a permanent part-time Clinical Partner - Nursing role and looks after clients living in the Central West Region. This is 3 days per week between Monday - Friday. You will use your car to drive between clients homes in the below areas:

Client Coverage Area:

• Lithgow
• Bathurst
• Oberon
• Cowra

As Clinical Partner - Registered Nurse, you'll be a valued member of our growing Home Care Clinical Care & Practice Team, working closely with an experienced, multidisciplinary team to provide tailored and individualised services to older adults living at home.

A typical day will involve assessing functional care needs, setting care goals, and providing treatment and intervention, helping people stay active, independent and right where they belong - home.

You'll be provided with a broad and varied caseload, with support from experienced clinicians. This meaningful role will include performing home visits or running group activities for clients accessing Support at Home or CHSP services or residing in one of our Seniors Living units. You will deliver person-centred care in an environment that promotes continuous quality improvement, consistent with the values of HammondCare and the Australian Health Practitioners Regulatory Agency (AHPRA).

About you

• You're a Registered Nurse with current AHPRA registration and practical experience, or sound knowledge in Aged Care, Inpatient, Community, Home-Based or Rehabilitation or related setting.
• You're a strong communicator, and able to plan and manage your time effectively.
• You enjoy working independently as part of a multidisciplinary team.
• You have access to a reliable vehicle, valid driver's license and safe driving record to travel between client's homes and HammondCare locations.
