Job summary

Juniper Sarah Hardy, located in Kelmscott, is seeking a Registered Nurse to join our supportive residential aged care team. In this role, you will assess residents’ clinical needs, coordinate care with healthcare professionals, and implement and review person-centred care plans.

You will also provide guidance to care staff and help ensure high standards of clinical care for residents.

Qualifications

• Current AHPRA registration as a Registered Nurse.
• 2+ years’ RN experience in aged care setting.
• Strong clinical assessment and care coordination skills.
• Ability to guide staff in wound and diabetes management.
• Sound understanding of medication systems.
• Excellent communication, teamwork and problem-solving skills.

Responsibilities

• Conduct clinical assessments and respond to changes in resident health and wellbeing.
• Develop, implement and review person-centred care plans with residents and families.
• Administer medications and provide clinical treatments per standards.
• Provide leadership, mentoring and guidance to care staff.
• Coordinate resident care with GPs and allied health professionals.
• Maintain accurate and timely clinical documentation and reporting.
• Support AN-ACC assessments and ensure accurate clinical documentation.
• Ensure compliance with Aged Care Quality Standards and policies.
• Participate in infection prevention, risk management and quality improvement initiatives.

About the role:

Juniper Sarah Hardy, located in Kelmscott, is seeking a Registered Nurse to join our supportive residential aged care team.

In this role, you will assess residents’ clinical needs, coordinate care with healthcare professionals, and implement and review person-centred care plans. You will also provide guidance to care staff and help ensure high standards of clinical care for residents.

We are looking for a compassionate and motivated Registered Nurse who is passionate about aged care and committed to delivering safe, high-quality care.

The suitable candidate must be available to work Monday to Thursday from 7:00am-15:00pm.

Key Responsibilities:

• Conduct clinical assessments and respond to changes in resident health and wellbeing
• Develop, implement and review person-centred care plans in partnership with residents and their families
• Administer medications and provide clinical treatments in line with professional standards
• Provide leadership, mentoring and guidance to care staff
• Liaise with GPs, allied health professionals and external providers to coordinate resident care
• Maintain accurate and timely clinical documentation and reporting
• Support AN-ACC assessments and ensure accurate clinical documentation
• Ensure compliance with Aged Care Quality Standards, policies and best practice
• Participate in infection prevention, risk management and quality improvement initiatives

Key Requirements:

• Current AHPRA registration as a Registered Nurse
• 2+ years’ experience as an RN in an aged care setting (or equivalent)
• Strong clinical assessment and care coordination skills
• Ability to guide staff in wound and diabetes management
• Sound understanding of medication systems
• Excellent communication, teamwork and problem-solving skills
