Job summary

Chandler Macleod is recruiting on behalf of our client for multiple Local Community Connector opportunities across Western Australia. This 12-month contract supports people with disability, their families, carers, and communities to access information, services, and supports.

You will build relationships, promote inclusion, and assist participants to connect with NDIS and community services. Role involves travel and community engagement across regional and remote areas, with responsibilities

Qualifications

Experience delivering high-quality customer service within community, government, health, or service-focused environments.
Ability to respond to enquiries, resolve issues, and provide accurate information.
Strong communication and stakeholder engagement skills.

Responsibilities

Provide information about disability supports, community services, and available programs.
Support participants and service providers by gathering and verifying information.
Assist with participant plan implementation, monitoring, and ongoing engagement.
Develop and maintain strong relationships with local community stakeholders.
Promote inclusive and culturally safe community participation.
Conduct participant check-ins and support evidence gathering activities.
Complete administrative tasks and maintain accurate records.
Participate in outreach activities within regional and remote communities.
