Chandler Macleod is recruiting on behalf of our client for multiple APS4 Local Community Connector opportunities across Western Australia.
This is an exciting 12-month assignment supporting people with disability, their families, carers, and communities to access information, services, and supports.
Working within a community-focused environment, you will play a key role in building relationships, promoting inclusion, and supporting individuals to connect with the National Disability Insurance Scheme (NDIS) and broader community services.
As a Local Community Connector, you will provide a visible, local presence within the community and support people to navigate available services and pathways. You will work collaboratively with participants, service providers, community groups, and internal stakeholders to promote participation, inclusion, and positive outcomes.
