Job summary

Ruah is seeking a Support Worker for the Stronger Ground Project in Mandurah to deliver personalised psychosocial support to people with severe mental health challenges. You will work one-on-one to identify goals, coordiate supports, and promote recovery, independence and social inclusion, reporting to the Team Lead.

This full-time permanent role is based in Mandurah. We value person-centred approaches, diversity and lived experience, with a comprehensive package including SCHADS Level 2 pay,

Qualifications

• Completion of, or progress towards, a relevant qualification (Certificate II or III), relevant work experience and/or lived experience
• Experience supporting people with mental health challenges and complex needs
• Ability to engage with individuals with diverse needs and build professional relationships

Responsibilities

• Provide personalised psychosocial support to individuals living with severe mental health challenges
• Identify goals with participants and develop recovery-focused support plans
• Assist with stable accommodation and independent living skills
• Support access to employment, education, health and community services
• Coordinate referrals to promote positive outcomes
• Build professional relationships with participants, carers and stakeholders
• Deliver strengths-based, recovery-oriented and person-centred support
• Maintain accurate client records and reporting
• Promote community participation and wellbeing
• Collaborate with mental health services and local partners
• Participate in supervision and ongoing professional development
