Job summary

Mission Australia is seeking a Recovery Case Worker to join our Community Living Support (CLS) team in NSW. You will provide psychosocial recovery-focused support to people living with severe mental illness, helping them develop skills and independence in their communities.

You will collaborate with participants, families, carers, clinical teams, and services to promote recovery, wellbeing, inclusion, and sustainable outcomes.

Qualifications

• Qualification in Mental Health or related field or working towards.
• Experience supporting people living with severe mental illness.
• Recovery-oriented and trauma-informed practice understood.
• Case management and support coordination skills.
• Current Driver Licence.
• Police history check and Working With Children/Vulnerable People clearance required.

Responsibilities

• Build positive, professional relationships with participants.
• Deliver psychosocial recovery-focused support tailored to goals.
• Facilitate planning sessions and develop individual support plans.
• Provide case management and care coordination to support recovery outcomes.
• Support independence and living skills development.
• Deliver psychoeducation and recovery-focused skill development.
• Assist access to community, health, housing and social services.
• Maintain communications with families, clinical teams, and stakeholders.
• Identify risks and respond per policies.
• Maintain case notes and reports.
