Job summary

Empower Recovery in Australia seeks a dedicated Case Manager to work with individuals on recovery, independence, and community integration. You will assess needs, coordinate services, and navigate resources to support meaningful outcomes.

Collaborating with consumers, supports, and providers, you will advocate for access to housing, healthcare, and employment, while documenting progress and maintaining compliance in a dynamic, client-centered environment.

Qualifications

• Bachelor's Degree in Human Services or a related field is required.
• Valid driver's license and reliable transportation.
• Intermediate computer skills and proficiency with electronic documentation systems.
• Ability to pass federal and state criminal background checks.
• Minimum of two (2) years of experience in human services or related field.

Responsibilities

• Assess participant strengths, needs, goals, and barriers to determine services and resources.
• Coordinate development, implementation, and review of recovery plans.
• Collaborate with participants, supports, and stakeholders to set realistic recovery goals.
• Monitor progress and adjust plans to support ongoing success.
• Provide community-based case management focused on recovery and skill development.
• Connect participants to healthcare, housing, employment, and other resources.
• Advocate for access to needed services and supports.
• Complete assessments and progress reports following program standards.
• Participate in on-call rotation for crisis intervention and support.
